The Court Service has amended Adoption form A58 for use from 3 October 2016. The amendments relate to the production of documents with the application for an adoption order. The new form supports an increase in the types of documents in relation to identity that the prospective applicant(s) can provide to the court to include a driving licence where applicable.

The amended version of this form is available in FormEvo and is for use from 3 October 2016. We are told that the notes to the form A58 Notes, remain unchanged at this time.
